This form is for parents and legal guardians of children who may be photographed or filmed during activities at Hope Reformed Baptist Church (HRBC). Please complete one submission per household unless you need different arrangements for different children.

I understand that HRBC, or people authorised by HRBC, may create still photographs, audio, and/or video of church activities in which my child(ren) may appear. I grant HRBC a non-exclusive, royalty-free licence to use, reproduce, and distribute such images and recordings for church-related purposes, including ministry promotion, encouraging the congregation, announcements, teaching, historical record, and reporting on church life.
Uses may include printed materials, the HRBC website, and official church social media accounts.
